Requesting a Copy of Your Records

A copy of your medical records may be obtained by downloading and printing the form below and bringing it to your doctor's office with a copy of your ID, Kent Hospital's medical records office is on the first floor of the main hospital or by mail.

In many circumstances an original authorization is required. For additional information about access to medical records, please call (401) 737-7000, ext. 34291. 

Please note, questions and requests for medical records should never be submitted via email.

Birth Certificates

There are two ways to request a copy of a birth certificate: by mail or in person.

By mail: Birth certificates can be obtained by writing to the Office of Vital Records, Room 101, Cannon Building, 3 Capitol Hill, Providence, RI 02908-5097 or to any city or town hall in Rhode Island. If you send for a birth certificate through the mail, you must include a photo copy of your driver's license.

In-person: Requests may be made in person at the state office address listed above or to any city or town hall in Rhode Island.

Please be advised that in most cases, you will receive your records more quickly from the city or town locations than from the State Office.
